Sleep and Women's Hormonal Health: Why Sleep Is Not Optional

Sleep is the most underrated hormonal intervention. A majority of women in India have inadequate or disrupted sleep — from demanding work schedules, family responsibilities, the blue-light culture of smartphones, and the cultural normalisation of sleeping fewer than 7 hours. The hormonal cost of this sleep deficit is substantial and specific.

What Happens Hormonally During Sleep

Sleep is not passive. It is a period of active hormonal regulation:

Growth hormone (GH): 60 to 70% of daily GH secretion occurs during the first few hours of sleep in pulses during slow-wave sleep. GH drives tissue repair, muscle maintenance, fat metabolism, and insulin sensitivity. Inadequate deep sleep suppresses GH significantly.

Cortisol rhythm restoration: Cortisol should be low at night (allowing sleep) and peak in the early morning (enabling waking and energy). This diurnal rhythm is disrupted by inadequate or disrupted sleep — producing elevated evening cortisol (insomnia), blunted morning cortisol (fatigue), and overall HPA axis dysfunction.

Insulin sensitivity: A single night of inadequate sleep reduces insulin sensitivity by 20 to 30% — equivalent to gaining several kilograms of weight. Chronic sleep deprivation is a significant, independent driver of insulin resistance.

Leptin and ghrelin (appetite hormones): Sleep deprivation reduces leptin (the satiety hormone) and raises ghrelin (the hunger hormone) — driving increased appetite, particularly for high-carbohydrate foods. This is a direct hormonal mechanism for the weight gain associated with poor sleep.

Thyroid: TSH peaks during the night. Disrupted sleep reduces TSH pulsatility and impairs thyroid hormone levels.

Oestrogen and progesterone: Sleep disorders are associated with menstrual irregularity, presumably through the interconnected effects of cortisol and GH dysregulation on the reproductive axis.


Conditions Particularly Linked to Sleep and Hormonal Health

PCOS and sleep apnoea: Women with PCOS have significantly higher rates of obstructive sleep apnoea (OSA) than women without PCOS — even when corrected for BMI. OSA disrupts sleep architecture, raises cortisol, worsens insulin resistance, and amplifies all PCOS metabolic features. Any woman with PCOS who snores, has excessive daytime sleepiness, or is significantly overweight should be screened for OSA.

Perimenopause and sleep: Hot flashes wake women repeatedly during the night. The resulting sleep fragmentation disrupts cortisol rhythm, worsens mood (perimenopause-related depression is partly sleep-disruption driven), and accelerates metabolic changes of the perimenopausal transition.

Postpartum sleep deprivation: Severe sleep disruption in new mothers is associated with elevated cortisol, insulin resistance, postpartum depression risk, thyroid dysfunction, and difficulty losing postpartum weight.


Improving Sleep for Hormonal Health

  • Consistent sleep and wake times — even at weekends. Consistent timing stabilises the circadian cortisol rhythm.
  • Light management: bright morning light (exposure to sunlight within 30 to 60 minutes of waking) sets the circadian clock; blue light avoidance (phone, tablet) 1 to 2 hours before bed reduces melatonin suppression.
  • Bedroom temperature: cooler temperature (18 to 20°C) improves deep sleep quality.
  • Avoiding caffeine after 2 PM (caffeine has a 5 to 6 hour half-life).
  • Stress management practices before bed (see stress blog).
  • For perimenopause-related sleep disruption: HRT is the most effective treatment for hot-flash-related sleep disruption.
