Exercise and Women's Hormones: The Type, Amount, and Timing That Matter

Exercise is one of the most powerful hormonal interventions available — but the relationship between exercise and women's hormonal health is not simply "more is better." The type of exercise, the intensity, the volume, and the recovery all determine whether exercise is improving or disrupting hormonal balance.

How Exercise Affects Key Hormones

Insulin sensitivity: Exercise is the most potent non-pharmacological insulin sensitiser. During aerobic exercise, muscle cells take up glucose through insulin-independent pathways. The effect lasts 24 to 48 hours — making regular (daily or near-daily) exercise significantly more effective than infrequent high-intensity sessions. Resistance training builds muscle mass, increasing the total capacity for insulin-stimulated glucose uptake permanently.

Cortisol: Moderate exercise produces a brief cortisol spike followed by a return to below-baseline (the cortisol "afterburn effect" that reduces overall stress load). Excessive high-intensity exercise — more than 1.5 to 2 hours of intense training daily, without adequate recovery — produces sustained cortisol elevation that becomes counterproductive.

Oestrogen: Moderate exercise reduces circulating oestrogen (beneficial for oestrogen-dominant conditions like endometriosis and fibroids). Extreme exercise (athletic training at competitive levels) excessively reduces oestrogen, causing hypothalamic amenorrhoea.

Testosterone: Resistance training (weightlifting) produces a brief testosterone rise in women — which supports muscle building, libido, and energy. Moderate regular weightlifting is beneficial for all women, including those with PCOS.

Thyroid: Regular moderate exercise improves thyroid hormone sensitivity and modestly raises T3 levels. Extreme caloric restriction combined with intense training can suppress T3 (the "low T3 syndrome" of athletes under-fuelling).


Exercise for PCOS

PCOS benefits enormously from a specific exercise pattern:

  • Resistance training (3 sessions/week): The most effective single type for PCOS — builds muscle mass (the primary site of insulin-stimulated glucose uptake), reduces insulin resistance long-term, and has the strongest evidence for reducing androgens in PCOS.
  • Moderate aerobic exercise (150 minutes/week): Cardiovascular benefit, acute insulin sensitivity improvement, mood improvement.
  • Avoid excessive cardio without adequate fuel: Very high-volume cardio combined with caloric restriction raises cortisol, can worsen HPA axis dysregulation in PCOS, and does not produce the sustained insulin sensitivity improvement of resistance training.

Exercise for Perimenopause

Perimenopausal women benefit specifically from:

  • Resistance training: Critical for preventing sarcopenia (muscle loss) and maintaining bone density. The fall in oestrogen after menopause accelerates both muscle loss and bone loss — resistance training is the most effective intervention to counter this.
  • Weight-bearing aerobic exercise: Walking, dancing, low-impact aerobics — maintains bone density.
  • Yoga: Specific evidence for reducing hot flash frequency, improving mood, and improving sleep in perimenopausal women.

When Exercise Disrupts Hormonal Health

Too much exercise without adequate fuelling: The "female athlete triad" — disordered eating, menstrual dysfunction, and low bone density — occurs when energy expenditure significantly exceeds caloric intake for a sustained period. The hypothalamus senses insufficient energy availability and reduces GnRH production — periods stop, oestrogen falls, and bone density progressively falls.

This is not exclusive to elite athletes. Recreational exercisers who significantly restrict calories while training heavily, and women who have started intense exercise programmes without proportionally increasing food intake, can develop menstrual irregularity from this mechanism.
