Gut Health and Women's Hormones: The Microbiome Connection

The connection between the gut microbiome and women's hormonal health is one of the most actively researched areas in reproductive medicine. What has emerged is that the gut is not just a digestive organ — it actively participates in hormone metabolism, oestrogen regulation, immune function, and even mood, all of which interact with gynaecological health.

The Estrobolome: The Gut Microbiome's Role in Oestrogen Metabolism

Oestrogen, after performing its functions in the body, is processed by the liver and conjugated (bound to glucuronic acid) for excretion via bile into the intestine. In the intestine, a specific set of gut bacteria — collectively called the estrobolome — produce an enzyme (beta-glucuronidase) that deconjugates the oestrogen, releasing it back into the active form for reabsorption into the bloodstream.

The balance of estrobolome bacteria determines how much oestrogen is reabsorbed versus excreted. A healthy, diverse microbiome maintains appropriate oestrogen recycling. A dysbiotic microbiome (an imbalanced one) can significantly alter this:

Overactive estrobolome (excess beta-glucuronidase): Excess oestrogen is reabsorbed rather than excreted — contributing to oestrogen excess. This may worsen conditions including endometriosis, fibroids, PMS, and PMDD.

Underactive estrobolome: Insufficient oestrogen reabsorption — potentially contributing to low oestrogen states.


Gut Microbiome and PCOS

Women with PCOS consistently show gut microbiome dysbiosis — reduced microbial diversity, altered ratios of specific bacterial families, and increased gut permeability ("leaky gut") compared to women without PCOS. This dysbiosis:

  • Promotes systemic inflammation (elevating inflammatory markers that worsen insulin resistance)
  • Impairs short-chain fatty acid (SCFA) production — SCFAs improve insulin sensitivity and reduce androgen production
  • Alters bile acid metabolism — affecting hormonal signalling

Whether dysbiosis causes PCOS or results from it remains under investigation — but the bidirectional relationship is clear.


The Gut-Brain-Hormone Axis

The gut produces approximately 90 to 95% of the body's serotonin — the key neurotransmitter affecting mood, sleep regulation, and appetite. Gut dysbiosis reduces serotonin production and is strongly associated with anxiety, depression, and mood instability. For women with PMDD or PMS, in whom mood disruption is already pronounced in the luteal phase, gut dysbiosis may amplify mood symptoms through reduced serotonin availability.


Practical Steps for Gut Health and Hormonal Balance

Diverse fibre intake: The gut microbiome feeds on dietary fibre — specifically fermentable prebiotic fibre. A diverse diet rich in different plant foods (at least 30 different plants per week is a useful benchmark from the research) supports microbial diversity. Indian food has natural advantages here: dal, rajma, chana, sabzis of many types, roti with atta, fruits — all contribute fibre variety. Eating the same limited set of foods every day reduces microbiome diversity.

Fermented foods (probiotics): Dahi (curd) is India's most accessible probiotic food. Regular dahi intake introduces beneficial lactobacilli into the gut. Other fermented foods: kanji (fermented carrot drink, traditional in North India), idli and dosa batter (naturally fermented), homemade achaar (traditionally fermented), lassi. Commercial probiotic supplements also available.

Reducing gut dysbiosis drivers:

  • Minimise unnecessary antibiotics — they significantly disrupt the microbiome
  • Reduce ultra-processed food, refined sugar, and artificial sweeteners — all negatively affect microbial diversity
  • Manage stress (stress directly alters gut motility and microbial composition through the gut-brain axis)
  • Prioritise sleep (as described in the sleep blog — sleep deprivation alters gut microbiome composition)

Calcium-D-glucarate: A supplement that inhibits beta-glucuronidase activity in the gut, potentially reducing oestrogen reabsorption in women with oestrogen-dominant conditions. Growing interest in PCOS and endometriosis management — discuss with your doctor before using.
